My father Poonamchand Ravat had bought the above mentioned policy in 1985. He had paid premium of 5,000/- per year regularly for the entire term. I am attaching the receipt of last premium paid herewith.

With great regret, I would like to inform you that he died a peaceful, natural death at home at the age of 70. I am attaching the photocopy of death certificate along with this letter.

As per, the policy details, I being the nominee am officially entitled to claim the insured amount. I request you to send claim forms to complete the procedure. If there is need of any other documents then kindly inform me.

I had taken Jeevan Surbhi Policy from your branch on 5th June 2015. I have paid quaterly premium of 3,000/- regularly till date.

At the time of taking policy, keeping my financial condition in mind I had selected quaterly payment of premium, but now due to my promotion I can pay yearly premium and I am desirous to do so.

As per my knowledge we can get rebate on yearly premium. Kindly keeping my advantage in mind change the mode of payment from quaterly to yearly mode and inform me the same when done.

I had taken a Life insurance policy from your branch on 5th April, 2015. I have paid premium regularly till June 2018. The premium amount was 5,000/- quaterly. I have not been able to pay premium after June 2018, due to other financial expenses and due to that you have closed my insurance policy. I received your correspondence regarding it.

At present I am working with Sagar Garment Company as office superintendent and I can pay the premium amount again. I would like to revive my above mentioned policy. I would like to know the following in this regard.

(1) The last date I can pay premium.

(2) Total amount of premium lo be paid.

(3) The amount of interest to be paid on the outstanding amount.

(5) Is there any scheme for interest waive off for females.
